Who are the caregivers covered by this Partnership and Referral Tool?
Caregivers are significant to the care recipients, and provide them with regular, unpaid support, accompaniment, or care. They may be family members, friends, colleagues or neighbours.

What kind of partnership is expected between caregivers and the professionals who complete the Tool?
Care and service partnership refers to a relationship between the patient, their loved ones and health and social service professionals (clinicians, managers or others), which is part of a dynamic process of interaction and learning, values experiential knowledge and promotes patient self-determination, for free and informed decision-making and the achievement of optimal health outcomes.* (Adapted from p. 11)

Here’s how you might present the Partnership and Referral Tool to the caregivers you work with:
“ I see that you support (often, always, etc.) your (spouse, mother, etc.) and I understand that you take good care of them. Your role as a caregiver is very important to us. If you’ll allow me, I’d like to take a few minutes to ask you some questions to see if you have any needs related to your role as a caregiver. Are you comfortable with that?”
With the caregiver’s permission, you can then send the completed and signed Tool to the organization that seems to best meet the needs expressed by the caregiver. If the caregiver does not wish to be referred, considers that they are able to meet their own support needs, or wishes to postpone this step, welcome them to do so by giving them the contact details of Caregiver Support and/or the Federation of Quebec Alzheimer Societies. Also, if the caregiver wishes to postpone this step, it would be a good idea to keep a record of it in the follow-up of their journey, so that you can come back to this discussion if necessary.
